Membership Donation

Send in a hard copy of your credentials, education and experience and we will evaluate your records and notify you when your membership is confirmed and you have complied with the requirements of membership or tell you what you need to qualify.

After you qualify, we will send you your certificate of membership when we receive your full donation pledge.

1. Professional Member Application evaluation, none refundable $40.00 pledge donation is applied to your membership donation if you are accepted.

2. Supportive Member Application evaluation, none refundable $10.00 donation is applied to your membership donation if you are accepted. All applicants for this type of membership must send in or e-mail a statement as to why you desire to support this association with donations and time as well as what your intentions are in providing assistance to the associataion, if accepted as a member.

3. Student Member Application evaluation, none refundable $5.00 donation is applied to your complete membership donation if you are accepted. All Student applicants must send in or e-mail a statement as to why you desire to become a student member and support this association and what you intend to do if you are seeking to become a professional member in support of this Association!

4. All persons who are accepted as members of this association will automatically become members of the Science of Wellness Society.

You may choose a Subdomain name (http://your name.scienceofwellnessociety, or for your own specific domain name it is an extra $12.00 donation/Month (pre-paid for one year) $124.00 hosting and setup. (http://www.yourname.com or org)

Please put your information on a disk so that it can be upleaded easily onto your page. If you can scan your pictures - personal and office building if desired onto a disk please do so. It will be a $5.00 per picture for scanning if not on a disk!
